Different types of muscle injury classifications have been presented in the literature, however, in sports medicine, they can be broadly classified as lacerations (when an external sharp body cuts through the muscle, for example during traumatic accidents), contusions (when an external body impacts against the muscle itself, for example in contact sports), and strains (when the muscle fibres are extended over their capacity, for example during an explosive sports action that requires high levels of muscle activation and stretch). Muscle strains are among the most commonly reported injuries in sports with a wide range of clinical presentation and reported symptoms based on the severity of the injury itself. Several classifications have been developed over the years (Munich consensus, ISMuLT classification, British athletics system, etc.), however, generally speaking, a three-grade classification is typically presented to the patient. Grade I represent the mildest type of injury that can be reported, with a small amount of torn muscle fibres, localised pain, and slightly reduced muscular functioning. The second grade includes injuries that involve a significant amount of muscle fibres, but continuity in the muscle can still be seen. Patients typically report significant pain, swelling, bruise, reduced strength, and limited functioning that is present during the activities of daily living. The third grade, the most severe one, includes injuries where a complete tear or avulsion (detachment of the muscle from the bone) is reported. Muscle injuries are strongly irrorated by blood supplies and display an elevated healing potential. After an injury, the damaged structures go through a sequence of healing stages (degeneration, inflammation, regeneration and fibrosis) that, if correctly respected and optimised, lead to the complete repair of the damaged muscle. In case of muscle injuries, our strong recommendation is to seek medical attention from a specialised facility that can quickly provide you with a precise diagnosis and rehabilitation plan. Malpractices throughout the recovery process, such as incorrect diagnosis (e.g.: underestimation of the condition), aggressive or delayed rehabilitation (e.g.: exercises strategy implemented too early\/late and not aligned with the muscle recovery stages), and a poor return to sport process, can significantly impact your short- and long-term recovery outcomes (e.g.: poor muscle quality repair, increased likelihood of relapses, decreased performance, etc.).
