The carpal bones are eight small bones that are interlinked and create the shape of the proximal part of the hand. The scaphoid is the biggest bone among the carpal bones, and it plays a fundamental role in forming the wrist joint. Falling onto an outstretched hand is the most common mechanisms of scaphoid fractures. These fractures can happen in all population groups, from kids to the elderly, athletes and the sedentary population. In subjects that are affected by specific conditions that make the bone more fragile (e.g.: osteoporosis), these injuries can happen with low-energy impacts, while in younger populations, higher-energy traumatic events are more common. Scaphoid fractures are relatively rare, however they are the most common among carpal fractures. Fractures to the scaphoid are unfortunately often misdiagnosed as a wrist sprain, and because of this, patients do not promptly commence a proper recovery plan. Due to its retrograde nature of blood circulation (different circulation from other structures in our body), a fracture to this bone can leave a part of the bone not well irrorated and exposed to potential complications in bone healing. Patients affected by this injury typically present pain localised on the site of the fracture that can radiate to other structures, swelling, bruises, and impacted wrist and thumb functioning. Certain scaphoid fractures (e.g.: small and stable fractures, located within the distal third of the bone) can be managed conservatively with immobilisation\/protection (through the implementation of a cast or a thumb splint) and rehabilitation. In case surgical treatment is deemed necessary, many different procedures are available and always discussed between the patient and the medical-surgical staff. After surgery, a period of immobilisation\/protection with the use of a sling, cast or brace is typically recommended and rehabilitation should start as soon as indicated by the medical team. Based on your rehabilitation needs, your recovery process will take place in four different environments, the pool, the gym, the movement analysis and retraining room and the pitch. In the early stages, the main goals are to restore homeostasis by reducing pain and swelling, recovering mobility, and gradually restoring wrist\/hand and upper limb functioning. In the mid-stage of rehabilitation, wrist\/hand strength recovery becomes the priority, together with the optimisation of the general movement quality to learn how to integrate the wrist\/hand and upper limb back into activities of daily living and sports.
